Address: 2602 College Dr, Phenix City, AL

What is offered: Pfizer and Moderna COVID-19 vaccines & booster shots
Who can come: All community members are welcome, but please see eligibility requirements from the CDC, listed below.
Required documents: driver’s license or other form of ID; vaccination card if getting a booster shot

Eligibility Requirements:  To get your booster shot, you must be at least 18 years old, and you must wait 6 months after your initial round of Moderna/Pfizer vaccinations, or wait 2 months after your initial Johnson & Johnson vaccination.  Additional eligibility requirements can be found at cdc.gov/COVID-19.  People who wish to get vaccinated for the first time must be at least 5 years old to receive the Pfizer vaccine at and least 18 years old to receive the Moderna vaccine.

Mixing and matching is allowed.
You may choose which vaccine you want for your booster shot, even if you got a different brand for your initial round of vaccinations.  Example: If you got vaccinated with Pfizer in March, you may choose to get Pfizer OR Moderna for your booster shot.

Why should you get a booster shot?
Initial vaccinations (two shots of Moderna or Pfizer, or one shot of J&J) continue to be highly effective in reducing risk of severe disease, hospitalization, and death due to COVID-19.  However, antibody levels may start to decline several months after initial vaccination.  Booster shots offer additional protection and can provide the immune system with a “boost” to promote higher levels of antibodies.  Go to cdc.gov for more information.

Optional: Reserve your spot!

To make sure we have enough doses for everyone, please email [email protected] with subject line “Vaccine Clinic Nov. 16” and specify if you will need Moderna or Pfizer.  You can also sign up with the QR code, or this link: https://airtable.com/shrARmcQdofWE7GMW.  Please arrive by 1:15pm at the latest and be prepared to wait 15 minutes after your shot for a standard observation period.  Signing up is optional – if you don’t get a chance to sign up in advance, you are still welcome to come get your shot!
